If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. Please ensure you have completed the travel form prior to booking an appointment with the nurse and you have given yourself plenty of time to get all your vaccinations done (please give at least 6 weeks to get an appointment with our Practice Nurses).

It is important to make the initial appointment as early as possible - at least 6 weeks before you travel. These vaccines have to be ordered as they are not a stock vaccine. Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Our HCA is fully qualified to carry out the following services:
1. Lifestyle checks, including blood pressure monitoring, lifestyle advice, advice on alcohol, smoking and weight.
2. Phlebotomy (taking your blood for analysis)
4. Weight management
5. Flu and pneumococcal vaccinations (cannot do children)
6. Vitamin B12 injections
7. Assisting the GP's in minor surgery, coil fits and implant removals

There are many things our doctors do during their own time as they are not core NHS services. This includes letters to outside agencies, fitness letters, medicals for employment or hobbies e.g. diving or racing driving, holiday cancellation etc. A full list of our fees can be found above. You will be informed of the charge beforehand and you will be expected to pay for the letter/medical prior to your consultation.
From 1st April 2015 the Government have asked all practices to ensure that patients be given the option to view their medical records electronically. Swanlow Medical Centre has been giving patients access to certain aspects of their records for over 5 years i.e. you have been able to book appointments and order your prescriptions on line. From 1st April we are taking this service further to allow you to view your medical records from the comfort of your own home.
The process is a three step process:
1) you register yourself for an online account via this website from the home screen
2) you come to the surgery with one photo ID and one other form of ID with your full name and address on e.g. utility bill, this is to ensure that we are giving access to the appropriate person.
3) we will activate your account once we are happy you are who you say you are and you have read and understood the rules around on line access.
If you would like to read and sign the below form and bring it to the surgery when you bring your ID this would save time at the reception desk.
