If you are travelling please ensure you leave enough time to get this as it is sent out via the postal service.
We CANNOT print off Covid Passports - they are either electronic on your app or on paper via NHS 119
We do NOT provide "proxy" access for children aged between 11 and 15 and NHS 119 will not provide a letter either.
Covid-19 Local Testing
Please come to the surgery and ask the Receptionist for a registration form to Register as a Patient. Alternatively you can register via this website on the home page.
These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.
Open the leaflets in one of the following languages
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.Pre-registration FormOnline medical questionnaire for new patients
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
Any new patient registering with us from 1st April 2015 aged 16 and over will need to fill in a Alcohol Screening Questionnaire. This is a Government led initiative which all practices have to undertake.
