My Planned Care

If you are currently waiting for a hospital appointment, operation or treatment, we wanted to let you know that the NHS has recently launched the My Planned Care Patient Digital Platform www.myplannedcare.nhs.uk which gives you direct access to the latest average wait time information for your trust, as well as helpful advice and support whilst you wait.
We encourage to look at this site where you may find the information you need before contacting your GP or hospital for an update about any elective procedures you are waiting for.
Updated weekly, the site is easy-to-use and is ‘open-access’ so your carer, friends, relatives and your NHS team can also access this information.

Wearing a face covering

With the recent change in government restriction guidelines to COVID-19, we ask that all patients attending the surgery continue to wear a face covering. This is for your protection as well as our staff.

Covid Passports

If you are aged 16 or over your Covid passport can be found on either the MyGP app, Patient Access app or the NHS app or if you do not have a mobile phone you can ring 119 and they will organise a letter to be sent to you.  If you are travelling please ensure you leave enough time to get this as it is sent out via the postal service.

We CANNOT print off Covid Passports - they are either electronic on your app or on paper via NHS 119

We do NOT provide "proxy" access for children aged between 11 and 15 and NHS 119 will not provide a letter either.

How to Register

Please come to the surgery and ask the Receptionist for a registration form to Register as a Patient. Alternatively you can register via this website on the home page.
